OLDER ADULTS HOUSE CALLS PROGRAM

How does the Older Adult House Calls Program collaborate with the patient’s primary care practice?
Older Adult House Call referrals are made through the patient’s primary provider office, YH walk-in or YH Emergency Department. Can I call to make a referral or send a telephone encounter?
